PRIVACY POLICY FOR SEAFORD OSTEOPATHY​

This document outlines Seaford Osteopathy’s Privacy Policy as of September 2021

 

Seaford Osteopathy is committed to providing you with the best possible customer service and protection of your private information. 

We are bound by the Privacy Act 1988 (Crh). The Act sets out the principles concerning the privacy of individuals.

 

Collection, use, and disclosure of your health Information:

 

Osteopaths must keep clinical records. It’s a government requirement, and information about your medical and family health history is needed to provide accurate diagnoses and appropriate treatment.

 

By providing Seaford Osteopathy with this information you consent to the collection, use and disclosure of information in the manner set out below.

 

We will only seek information that is relevant to your health. This information is generally collected directly from you or otherwise with your consent. 

This information may be collected via; our online booking service (Cliniko), fax, email, mail, over the telephone or in person in the clinic.

 

Seaford Osteopathy does not share or disclose any of your personal information with out your verbal or written prior consent.

 

Examples of who we may disclose your information following your consent include; other practitioners at Seaford Osteopathy, GPs, medical specialists, radiologists, pathologists, other allied health practitioners, gym and fitness instructors. 

 

At your request, your health information held by this practice will be made available to another health service provider. 

 

There are circumstances where a health practitioner is legally required to disclose information. An example of this is a subpoena or other mandatory reporting requirements.

 

 

We use Cliniko to enable bookings (online via our website) and for the secure storage of your medical records. Cliniko’s Privacy Policy can be viewed here: https://www.cliniko.com/policies/privacy/ 

 

Whenever your data is entered in Cliniko, it’s encrypted using HTTPS (end-to-end encryption). Cliniko use a 2048-bit SSL certification for encryption in transit. All data is also encrypted at rest and backed up daily, using the industry-standard AES-256 encryption algorithm.(https://www.cliniko.com/security/)

 

You have the right to access your information and medical records. You may ask to view or receive a copy of your record. A processing cost may be involved in accordance with Australia’s Privacy Guidelines.

If you believe that any personal information about you is incorrect or out of date, you are entitled to ask us to correct it.

 

We will inform you if we intend to transfer your clinical records to another osteopath or clinic, or if we sell or close this clinic.
